The HWPM will work to maintain professional integrity, exemplifying and prompting the mission, vision and values of the organization, as follows: Mission to restore mothers and families through a comprehensive diversion program of treatment, recovery, and hope. Vision intergenerational cycles of incarceration, addiction, and poverty no longer exist. The Primary Duties of this Position: The HWPM will work with the Program Director to develop, implement and evaluate health programming, facilitate and oversee biometric assessments, as well as create individual health and wellness plans for clients based on SMART goals. This position will provide programming for chronic disease prevention and health promotion, including but not limited to tobacco cessation, physical activity, nutrition, and sexual health and wellness, serve as the primary advocate for clients medical needs, and manage, sustain and develop community partnerships in areas of health. 1 | P age Health and Wellness Program Manager Duties and Responsibilities of this Position: Utilize evidence-based curriculum to educate and promote healthy living aspects such as tobacco cessation, sexual health, healthy relationships, and more Perform basic biometric and physical assessments for clients to determine height, weight, glucose, cholesterol, triglycerides, BMI, % body fat, circumference measurements, and physical activity measurements to assist participants in understanding their current health Assess and address motivational and psychosocial issues in relation to health, such as makers for eating disorders and food addictions, behavioral or chronic issues that may prohibit growth in health and wellness. Communicate and work with ReMerge team to action plan or refer clients for services to address needs Create and monitor health and wellness plans for each client based on assessments and observed needs quarterly. Meet with clients individually as needed to coach them on meeting their health and wellness plan goals Track clients health and wellness progress, ensuring they are meeting the health competencies outlined. Make recommendations for progress plans to guide client progression when competencies are not met Organizational meal planning and overseeing the nutrition program to include ordering food and working with clients to prep meals daily, as well as oversee kitchen stocking and cleanliness Ensure data compliance for all participant and grant needs Serve as advocate for medical needs of clients and insure compliance with ReMerge guidelines regarding medical releases of information, liability forms, prescription management forms, and HIPPA Manage and upkeep the ReMerge raised garden beds Transport participants as needed Oversee health and wellness budget to include development and regular review Oversee health and wellness volunteers Develop and/or implement participant and employee wellness initiatives quarterly Oversee medication compliance and communicate with team when noncompliance or side effects arise Serve as the primary contact to assess and triage all medical issues, including ER and urgent care visits and needs Education and Experience Required: Bachelors Degree in the field of Health Promotion, Public Health, Health Education, Nutrition Science, Psychology, or related field with 1-2 years experience. Masters Degree is preferred. Experience can be substituted. Knowledge of addiction and recovery, case management, social services, criminal justice system, and/or justice reinvestment initiative is preferred. Knowledge, Skills and Abilities Required: Significant knowledge and practice of public health is a must and personal experience helping high-need participants achieve employment milestones is preferred. High degree of empathy and ability to meet participants where they are as well as to help them envision where they could be A general knowledge of community health resources and collaborative work initiatives in the Oklahoma County area 2 | P age Health and Wellness Program Manager A basic knowledge of coordinated approach to health and wellness in the following sectors: physical education and physical activity, nutrition environment and services, health education, social and emotional climate, physical environment, health services, employee wellness, community involvement and family engagement. Knowledge and experience coordinating with team members, organizations and community agencies on prevention programs, management information process, grant tracking, social and health services system development, program selection criteria and evaluation is needed. Excellent verbal, interpersonal, writing and communication skills Ability to work in a fast-paced environment and ability to prioritize tasks and duties for self and others Ability to work in a variety of settings with culturally diverse individuals with the ability to be culturally sensitive and appropriate.
